ACKNOWLEDGMENT OF RISKS

 I, the undersigned person request ,I am aware I will be crawling in relatively small passages, climbing up and down ladders, walking and crawling over loose and slippery rocks and walking through muddy passageways. Furthermore, I understand that certain risks and dangers exist in this activity which may result in serious injury. I am aware that in signing this document on my behalf, for participation in ISV20 Field Trip that certain elements of the activity are physically and mentally demanding.

EXPRESS ASSUMPTION OF RISK AND PERSONAL RESPONSIBILITY

I confirm that I am physically and mentally capable of participating in the activity. I knowingly and freely assume all such risks, both known and unknown and assume full responsibility for my participation. I confirm that I will stay within permitted areas, within my group and in close contact ISV 20 guide and/or staff members during my time in the caves.

RELEASE OF LIABILITY

In consideration of the service and facilities provided, I, for myself;, do hereby release, waive, and discharge the organizator, Dak Nong UNESCO Geopark, their officers, employees, principals, directors, agents, and volunteers from any and all liability to the undersigned’s personal representatives, assigns, heirs and next of kin, for any of any and all injury, disabilty, death, or loss or damage, to personal property.

SCOPE OF RELEASE AND INDEMNITY

I have read this document and I understand it is a release of all claims. I understand and assume all risk inherent. I voluntarily sign my name evidencing my acceptance of the above provisions. I certify to have a specific insurance covering my travel in Vietnam as well as the ISV20 field trip and activities

If you arrive to Ho Chi Minh city or Buon Ma Thuot city before Nov, 21th, please provide your address, we shall pickup within 5km from the main route.

FOR SYMPOSIUM ONLY

If you stay extra days in Ho Chi Minh, please provide your hotel address, we will drop-off within 5km from city center.

* Important notice: Short Fieldtrip package only covers accommodation for night of the 24th and Main Fieldtrip package covers accommodation for nights of 24th and 25th. Early registration will have priority to choose hotels in Krong No District, late registration will have to stay in Buon Ma Thuot with extra fees. In order to maximize the capacity of stay in Krong No District, those choose twin room but travel alone needs to share room with others (same gender).
